%WW %./ <br />ADVERTISING COMPANI - ADDITIONAL SUBSURFACE INVESTIGATION HEALTH AND SAFETY PLAN <br />2.4 HEALTH AND SAFETY RESPONSIBILITIES <br />Site activities will entail field operations involving Earth Tech and/or subcontractor personnel. This <br />section describes the responsibilities, lines of authority and communication which will be established at <br />the work site with regard to health and safety. <br />2.1 ALL PERSONNEL <br />.. Each person is responsible for his/her own health and safety, for completing tasks in a safe manner, and <br />for reporting any unsafe acts or conditions to his/her supervisor and/or the Site Supervisor. All personnel <br />are responsible for continuous adherence to these health and safety procedures during the performance of <br />�„ their work. No person may work in a manner that conflicts with the letter or intent of safety and <br />environmental precautions expressed in these procedures. After due warnings, Earth Tech will dismiss <br />from the site any person who violates safety procedures. Earth Tech employees are subject to progressive <br />discipline and may be terminated for blatant or continued violations. All onsite personnel will be trained <br />in accordance with 8 CCR 5192(e) 29CFR 1910.120(e), and this document. <br />4.. <br />2.2 PROJECT MANAGER (Mr. Roy Litzenberg, R.E.A.) <br />The Project Manager (PM) has overall management authority for ensuring that all project activities are <br />completed in accordance with requirements set forth in this plan. The Project Manager will confer with <br />the designated Health and Safety Professional on all matters affecting health and safety. Other health and <br />safety related duties of the PM include: <br />1. Reading and becoming familiar with this HASP. <br />2. Requiring a prompt and thorough investigation of all accidents. <br />%W 3. Providing day-to-day management of site work activities. <br />2.3 HEALTH AND SAFETY PROFESSIONAL (Mr. Johnathan Moore, C.S.P.) <br />The designated Health and Safety Professional is responsible for overseeing all aspects of the site safety <br />program, and preparing any site-specific safety guidance documents or addenda to this plan. The <br />designated Health and Safety Professional does not report to the Project Manager, and is separately <br />!� accountable to Earth Tech senior management for site health and safety. The Health and Safety <br />Professional will act as the sole contact to all regulatory agencies on matters of safety and health. The <br />Health and Safety Professional's other responsibilities include: <br />t� <br />1. General health and safety program administration. <br />4., 2. Conducting project health and safety audits as warranted. <br />3. Developing site-specific employee/community emergency response plans, as required, based <br />on expected hazards. <br />4. Determining the level of personnel protection required. <br />r <br />4W 5. Updating equipment or procedures based on information obtained during site operations. <br />6.
