r,W <br />1%� SWOOO <br />NATIONAL ADVERTISING COMPANY— ADDITIONAL SUBSURFACE INVESTIGATIW, HEALTH AND SAFETY PLAN <br />2.4 SITE SUPERVISOR/SITE SAFETY OFFICER (Mr. Rod Lazo) <br />During any site activities the work will be managed by a Site Supervisor, an Earth Tech employee who <br />will exercise direct responsibility for implementation of the HASP during these work operations. This <br />responsibility includes communicating site requirements to all personnel, observing that field personnel <br />L" and/or subcontractors enforce all provisions of this HASP, and consulting with the Health and Safety <br />Professional regarding changes to the HASP. <br />16W The Site Supervisor or designated alternate will serve as the Site Safety Officer (SSO) at the work site. <br />The SSO is responsible for performing the routine duties for health and safety, with the coordinating any <br />necessary assistance from the designated Health and Safety Professional, The SSO will administer this <br />1., HASP and the applicable site-specific safety guidance document. Additional SSO responsibilities include: <br />1. Reading and becoming familiar with this HASP. <br />2. Enforcing the requirements of this HASP and other applicable safety requirements. <br />3. Stopping work, if necessary, to prevent injury or illness and ensure personal and <br />environmental health and safety. <br />4. Determining evacuation routes, and establishing/posting local emergency contact telephone <br />numbers. <br />5. Ensuring that all site personnel and visitors have received the proper training and medical <br />*00 monitoring before entering any controlled areas. <br />6. Presenting any tailgate safety meeting and maintaining appropriate training <br />im documentation/attendance records. <br />7. Implementing air monitoring according to directives in this Health and Safety Plan <br />' S. Implementing changes in health and safety procedures as directed by the Health and Safety <br />Professional and/or approved addenda to this HASP. <br />1. 2.5 SUBCONTRACTORS <br />Any Earth Tech subcontractor is responsible for assigning specific work tasks to their employees, and for <br />ensuring that their personnel are properly trained and participate in health and safety programs which <br />fulfill the requirements specified in this HASP (e.g., hearing conservation). Each subcontractors <br />management will provide qualified employees and allocate sufficient time, materials and equipment to <br />safely complete assigned tasks. In particular, each subcontractor is responsible for equipping its <br />personnel with any required personnel protective equipment. <br />Earth Tech considers each subcontractor to be an expert in all aspects of the work operations for which <br />L. they are tasked to provide, and each subcontractor is responsible for compliance with those regulatory <br />requirements which pertain to those services. Each subcontractor is expected to perform its operations in <br />accordance with its own unique safety policies and procedures, to ensure that hazards associated with the <br />y performance of the work activities are properly controlled. Copies of any required safety documentation <br />for a subcontractor's work activities will be provided to Earth Tech for review prior to the start of on-site <br />activities, if required. In the event that subcontractor procedures/requirements conflict with requirements <br />LW specified in this HASP, the more stringent guidance will be adopted. <br />Hazards not listed in this HASP but known to any subcontractor, or known to be associated with a <br />subcontractor's services, must be identified and addressed to the Earth Tech PM or Site Supervisor prior <br />hslprojsuplcominachasp.doc 2-2 02/09/98 <br />Lo <br />
