J NATIONAL ADVERTISING COMPANY— ADDITIONAL SUBSURFACE INVESTIGATION HEALTn AND SAFETY PLAN <br />4.0 GENERAL HEALTH AND SAFETY OPERATING PROCEDURES <br />The following requirements shall apply for all personnel performing any onsite work operations. <br />4.1 MEDICAL SCREENING AND HEALTH SURVEILLANCE <br />In accordance with Earth Tech Health and .Safety Policy HS601, Medical Surveillance,. all personnel <br />performing work activities inside any controlled .work: area (see Section.10.1) shall have completed a <br />_ physical exam in accordance with the requirements of 29'CFR 1910.120 --[fl, and .8`CCR 5192(f). Exam <br />procedures and tests will be specified by the examining physician. <br />tf <br />/ <br />The results of medical examinations are -to be evaluated 'by a physician specializing in occupational <br />medicine. The medical evaluation includes a judgment of the employee's ability to use respiratory <br />protective equipment and to participate in hazardous waste site activities. The examining physician <br />must document his evaluation/recommendations in writing. Restrictions of onsite activities may be <br />required for personnel with certain medical conditions which could be aggravated by chemical exposure <br />or physical demands at the site. 'Each- employee is responsible for notifying gthe Health and Safety <br />Professional of physical or medicalrestrictions. The Health and Safety Professional will then ensure that <br />project management observes and enforces the .restrictions, A copy of each persons written medical <br />evaluation will be made available for review following a request from the Health and Safety Professional. <br />Employees who have not received a medical examination within. 12 months (365 days) of their previous <br />medical exam will be required to immediately obtain an appropriate medical exam and provide a copy of <br />the medical evaluation to the Health. and. Safety Professional for review prior to starting work on the <br />project.-` <br />Personnel .who are not "routinely � exposed ,site hazards may be waived from the annual medical <br />examination requirement 6i the Health and Safety Professional provided that they meet the following: <br />• Medical examinations are; received. on a frequency determined by the examining physician, <br />but not to exceed everytwo (2) years' €' <br />r. <br />• Site activrties performed by the exempted personnel d6 -not entail exposure to contaminants In <br />3,1,: excess of the permissible exposure limits or other exposure limitations as outlined in;this <br />31 HASP: <br />A <br />4.2 TRAINING REQUIREMENTS <br />Since the work actions at the site are" considered part of activities at an uncontrolled waste site, the <br />! .,.requirements for training specified in Earth Tech Health and Safety�Pioceduce HS301, HAZWOPER <br />�. Training and Refresher, will apply to all work activities. <br />4.2.1 HAZWOPER TrainingIz <br />i ~n.ri331: <br />t.t; # ; <br />In --compliance with HS301, all field personnel or visitors involved with 'site'actiwities.'wi1! have <br />completed the necessary health and safety training courses prior to entering any controlled areas of the <br />site (Exclusion or Contamination Reduction zones). This includes, at a minimum, successful completion <br />F of training meeting: the., provisions established, in 29 CFR -.1910.120 (e)(2) and (e)(3) and 8 CCR <br />z' 5192(e)(2) and (e)(3) (40 -hour or 24-hour initial training). All personnel will also .:receive annual <br />refresher training'in accordance with 29 CFR ]91.0.120,(e)(8) and 8 CCR 5192(e)(8), and shall have <br />completed the most recent training course within the previous 365 days.
