C <br />C <br />L <br />L <br />L <br />L <br />1ATIONAL ADVERTISING COMPANY= ADDITIONAL SUBSURFACE INVESTIGATION HFALTII AND SAFETYPLAN <br />Electrical Receptacles. Receptacles .for attachment plugs shall be -of the approved, dead -front, <br />concealed contact type. Where different voltages, frequencies, or types of current are supplied, <br />receptacles shall be of such design that attachment plugs are not interchangeable. <br />Wet Environments. Work done in wet environments shall require ground fault interrupters and <br />water -tight connectors. <br />If maintenance of electrical systems is required, the equipment will be -de-energized and locked -out using <br />an approved lock -out device. The lock will be removed only be the personperforming the maintenance <br />work. <br />4.7 ACCIDENT OR INCIDENT REPORTS <br />The following procedures meet all requirements of Earth Tech Health and SafetyProcedure HS203, <br />Accident Reporting, Investigation and Review. <br />Personal. Accidents/Injuries. All accidents and incidents involving personnel, (Earth Tech or <br />subcontractor) that occur on site during field activities associated with this project will be promptly <br />reported to the Project Manager or SSO, who will notify the Health and Safety Professional. The <br />supervisor of the injured employee or work crew where the accident occurred will initiate a written <br />report. The Injury Report. form found :in Appendix B will be used, and must" be .fully completed to <br />ensure that all relevant information is recorded. The .Project Manager or Field Supervisor must <br />complete the "Manager" section of the report(s) and forward to the Health and Safety Professional. <br />The Health and Safety Professional will review the documentation, and will assist in the performance <br />of any necessary accident investigation' or other follow-up. The Project Manager will ensure that the <br />recommendations resulting from any investigation are implemented without delay. <br />Vehicle Accidents. Any motor vehicle incidents shall be reported immediately to the Health and <br />Safety Professional. Supervisors of personnel that `have been involved in a vehicle accidents or <br />incident that results in damages to equipment of vehicles in excess of $500.00 will complete a <br />Supervisor's Report of Incident and ensure that.the employee completes a Vehicle Accident Form <br />(see Appendix B) both of which will be submitted to the Health and Safety Professional. Motor <br />vehicle incidents that occur on'public:highways are normally investigated by the responsible law <br />enforcement agency; copies of these reports should be obtained by the driver and forwarded to' the <br />Health and Safety Professional as soon as possible. <br />4.8 VISITOR CLEARANCES <br />Visitors will not be allowed within any controlled work area (see Section 10.1) unless they comply with <br />the health and safety requirements.of this HASP, and can demonstrate an acceptable need for entry into <br />the work area. All visitors (including the -site owner or the owner's representative, -regulatory agency <br />representatives or Earth Tech clients) desiring to enter any controlled work area must observe the <br />following procedures: <br />A written confirmation must be received by Earth Tech documenting that each of the visitors has <br />received the proper training and medical monitoring required by this HASP. Verbal confirmation <br />can be considered acceptable provided such confirmation is made by an officer or other <br />authorized representative of the visitor's organization. <br />2. Each visitor will be briefed on the hazards associated with the site activities being performed and <br />acknowledge receipt of this briefing by signing the appropriate tailgate safety briefing form. <br />If the site visitor requires entry to any Exclusion Zone, but does not comply with the above requirements, <br />all work activities within the Exclusion Zone must be suspended and monitoring using direct reading <br />hslprojsupkominachasp.doc 4-5 02/09/98 <br />
