I have performed a <br />preliminary review of your site safety plan which includes a section entitled "scope of <br />operations". In summary you have proposed that you will replace the 15,000 gallon UST <br />with an identical UST. You have also propsed that the 10,000 gallon UST which is currently <br />out of round will be relieved of back fill pressure and measured for roundness. If it returns <br />to round (i.e. repaired) it will be returned to the excavation. If it the deflection remains <br />greater than that which is accepted by the factory then a new UST will be installed. <br />Please be aware that PHS/EHD policy 94-009 states that maximum time for a preliminary <br />review is 20 working days and maximum time for a full review is 40 working days. This <br />preliminary review is not to be considered by you as complete review. Under the conditions <br />of the repair permit section 5 you must "Attach detailed information as to methods used to <br />line/repair the tank... " The detailed information or repair work plan must be signed and <br />submitted by the licensed contractor. Submittals from non -licensed sub contractors will not <br />be approved by PHS/EHD. <br />The following is a list of issues that are not addressed in "Section 3.0 Scope of Operations" <br />contained in your site safety plan. These issues need to be addressed and the work plan <br />approved prior to scheduling any appointments at the site: <br />1) All removed UST's shall be transported to a licensed treatment or disposal facility. <br />Please specify the destination of the UST's. The treatment or disposal facility must <br />submit a manifest to PHS/EHD stating that it received the UST. <br />2) The method used to inert the tanks is not specified in the work plan. <br />3) Please submit the DTSC Tank Closure Certification Form to the inspector on site <br />prior to removal of either UST. <br />4) A removal permit or approval for the removal from Tracy Fire Department must be <br />provided with the work plan. <br />
