5) Section 9 of the repair permit states that a third party evaluation of the lining/repair <br />procedures shall be submitted to this agency. You have not submitted the report of <br />the assessment of either tank to this agency. In order to add the 10,000 gallon UST <br />to the repair permit the measurements of the tank deflection being greater than that <br />which will be warrantied by the manufacturer must be submitted by the <br />manufacturer to PHS/EHD. <br />6) Your work plan states that the 10,000 gallon UST will be tested on site to see if it will <br />return to the proper amount of deflection. The determination that the UST returns <br />to the proper amount of deflection acceptable to the manufacturer shall be <br />accomplished by measurements taken by the manufacturer. This report shall be <br />submitted to PHS/E1-1D prior to the UST being re -installed. <br />7) The method of de -watering the excavation and the disposition of the generated <br />wastewater. <br />8) The rinsate generated from cleaning the UST's is a hazardous waste and is to be <br />manifested off site. The hazardous licensed hazardous waste hauler for the repair <br />project has not been identified in the work plan. <br />9) The UST's are to be installed according to the manufacturer's installation <br />instructions by the licensed contractor. The compression measurements of the newly <br />installed UST's must be taken by the manufacturer and the manufacturer must <br />submit an independent report that the UST's have been properly installed to <br />PHS/EHD prior to the UST's receiving any fuel. <br />10) Once the UST's are installed all associated testing of all components of the entire <br />UST system will be required including testing of the vapor lines by APCD prior to <br />issuance of the operating permit by PHS/EHD. <br />11) The newly installed UST's will be installed according to the original set of plans <br />submitted by the licensed contractor and approved by this department on February <br />22, 1998. <br />12) Changes to the approved version of the repair work plan must be submitted in <br />writing to PHS/EHD. Deviations from the work plan in the field will require job <br />shut down until those changes can be evaluated by PHS/EHD and approved in <br />writing.
