4) Section 3 states "If either the vacuum or pressure test fail in the last part of Step 2, <br />then it is evident that the secondary wall is leaking water into the annular space" <br />PHS/EHD recommends removal of the UST if either the vacuum or the pressure <br />test fail. PHS/EHD recommends that all openings to the tank be fitted with tamper <br />proof covers during the pressure and vacuum tests. Note that PHS/EHD has final <br />approval for all repairs and conditions. This tank will not be approved until such <br />time as the tank shows that it is not leaking. <br />5) Section 4 states "a reasonable pressure of about 3 psi in the annular region only" <br />PHS/EHD recommends once the tank is removed to pressurize the annular space <br />per standard installation procedures by bleeding over from the primary to the <br />secondary to a pressure of just below 5 psi. The 5 psi will be maintained for lh hour <br />during which time the entire surface of the secondary tank is washed with a soap <br />solution. The presence of bubbles will indicate a crack in the secondary tank at which <br />point PHS/EHD recommends that the tank be repaired or replaced. <br />6) The Special Inspector must make all internal inspections and certification of all <br />repairs. <br />7) If the tank has been determined not to be damaged it will be required to have a <br />vacuum test of 7 inches of mercury held for 24 hours. Full requirements for this test <br />will be provided. Vacuum test is required for final approval. <br />8) The Underwriters Laboratory Label must be maintained and all warranties extended <br />by the tank manufacturer. With a letter to PHS/EHD and tank owner upon <br />completion verifying the above requirements. <br />9) PHS/EHD must witness all tests and inspections of the tank. If PHS/EHD is not at <br />the site the tests will not be approved. <br />10) The Special Inspector will provide written repair plans and certification prior to any <br />repair and will provide certification letters after the repairs are made. <br />11) PHS/EHD requires copies of all manifests for removed wastes. <br />12) A passing tank integrity test is required prior to issuance of the UST operating <br />permit. <br />
