The contingency plan stated that there were no local arrangements <br /> made in case of an emergency. After discussions with John Sanchez, the Maintenance Manager, he stated that there <br /> were local arrangements made with a clean up company, hospital and fire department. An evacuation plan was not <br /> available for the main maintenance shop or the outside chemical storage areas. The contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non-sudden <br /> release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br /> State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br /> others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill <br /> the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> This is a Class II violation. <br /> 1 CCR 66265.16(d) Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste were incomplete. The training records did not <br /> include job title for all employees and a job description for every job title related to hazardous waste. The records also <br /> did not state the type of training required for each position. Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to each <br /> person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former employees <br /> shall be kept for at least three years from the date the employee last worked at the facility. Ensure that employee <br /> training is properly documented. <br /> This is a Class II violation. <br /> Page 4 of 6 <br />
