There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator, location of fire extinguishers, <br /> spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has <br /> a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form <br /> is provided that can be used for this purpose. <br /> This is a repeat violation, Class II. <br /> 108 CCR 66262.23(x)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest number 003324936SKS (8/27/12) was not sent to DTSC within 30 days of shipment of <br /> hazardous waste and was not found on the DTSC hazardous waste tracking system website list for this facility's EPA <br /> ID number. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California <br /> is either the generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of the manifest listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. <br /> This is a Class II violation. <br /> 202 HSC 25189.5(a) Disposed or caused disposal of HW at an unauthorized point. <br /> White powder was observed accumulated around the drain in the lead acid battery storage room. The drain cover was <br /> observed to be on the side of the drain and not covering the drain. According to Mr. Alberto Zamora, the white <br /> substance smelled like battery acid. He stated that the drain led to the oil-water separator, which is managed and <br /> disposed of as non-hazardous waste. Hazardous wastes shall be disposed of only by transportation to a permitted <br /> hazardous waste treatment, storage, and disposal facility (TSDF).
