CCR, section 66262.40 (a)—Failed to keep signed copy for 3 years. <br /> Signed off TSDF manifest copies could not be found for the following manifests: <br /> 000186806GBF from 07118/07 <br /> 000302096GBF from 10/09107 <br /> 001700736JJK from 03/13/07 <br /> 002004047JJK from 03/28/07 <br /> 002004067JJK from 04/03/07 <br /> Provide copies of the signed off TSDF copy for the above manifests. Correct by <br /> 07/01/09. Class II Violation <br /> #89. CCR, section 66261.7 (f) — An empty HW or Haz Mat container (larger than 5 <br /> gallons) was not marked with the date it was emptied/not managed within one year of <br /> being emptied. <br /> (41) Empty contaminated 55-gallon containers and (40) Empty contaminated bulk <br /> containers were found onsite and were not marked with the date they became empty. <br /> Either provide a copy of a receipt for proper disposal or documentation that the <br /> containers have been labeled properly. Correct by 07101/09. Class II Violation <br /> SUMMARY OF VIOLATIONS: <br /> #5, #10, #15, #19, #25, #27 Y are Class II violations. <br /> Submit the Certification of Return to Compliance Statement, a corrective actions <br /> statement and supporting documentation(forms, receipts, reports, photos, etc.). <br /> Submit all required information as stated above by July 1, 2009. <br /> Note: Violations corrected onsite include, properly labeling(5) 55-gallon drums of waste <br /> filter cake sludge and (1) bulk container of used oil. <br /> Please call our unit secretary to enroll in our FREE CUPA Compliance Class at (209) <br /> 468-3427. <br /> All Environmental Health Department staff time associated with failing to comply by the <br /> above noted compliance dates will be billed at the current hourly rate. <br /> This facility is subject to reinspection at any time at the Environmental Health <br /> Department's current hourly rate. <br /> - 2 - <br /> Inspector: VIA) F4` Received By: <br />
