The following is an itemized list of hazardous waste violations that have not been <br />addressed for ASSOCIATED TRACTOR SVC INC as of August13, 2018. <br />Open violations from July 05, 2018 inspection <br />Violation #605 - Failed to completely label containers or portable tanks of hazardous waste. <br />Facility Yard: <br />-One 5 gallon metal pan containing used oil (90 percent full) was observed with no hazardous waste label. <br />-One blue 55 gallon steel drum and one white 55 gallon poly drum containing used antifreeze (1/4 full) were <br />observed missing the hazardous properties, physical propertiers, accumulation start date, and the name and <br />address of the generator. <br />-One blue 55 gallon steel drum containing used oil (1/4 full) was observed with no hazardous waste label. <br />Facility Shop: <br />-One steel drip pan next to a drill press contianing used oil (1/4 full) was observed with no hazardous waste label. <br />-One blue 5 gallon plastic bucket containing used oil (1/3 full) was observed with no hazardous waste label. <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. Submit a corrective action statement to the EHD once the containers have been labeled. <br />Violation #608 - Failed to properly manage used oil and fuel filters. <br />-One black 55 gallon drum containing used oil metal filters and used oil was observed open without a label. <br />-One black and one white 5 gallon plastic bucket was observed containing used oil filters (metal) were observed <br />with open without a label. <br />-One black 55 gallon steel drum containing punctured oil filters was observed without accumulation start date. <br />Used oil filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as "drained used <br />oil filters," and marked with an accumulation start date. Immediately properly drain, label and close the containers. <br />Ensure that all used oil filters are managed in this way. Submit a corrective action statement to the EHD once this <br />has been completed. <br />Bills of lading for the recycling of used oil filters for 2017 were not found on site. A copy of each bill of lading must <br />be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters for the last <br />three years and maintain them on site. Submit copies of the 2017 year to the EHD for review. <br />Open violations from August 12, 2016 inspection <br />Violation #104 - No modified contingency plan. <br />An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br />emergency coordinator on site or on call to coordinate emergency response measures, and the following <br />information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br />extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br />unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br />information by a phone. A form is provided that can be used for this purpose. <br />Page 3 of 5 <br />
