#ENTORY CERTIFICATION <br /> Instructions <br /> In order to simplify business compliance with the annual inventory reporting requirements of Chapter 6.95 of <br /> the California Health and Safety Code,we are providing an Inventory Certification Form. Businesses may use <br /> this form to meet this year's reporting requirement for all hazardous materials which are being handled in the <br /> same basic quantities as indicated in their last inventory submittal. <br /> Significant changes in your inventory,however,must be reported. A significant change is defined as a change <br /> of more than 100 percent in the quantity of a hazardous material handled at any one time from the previous <br /> years report. In addition, any hazardous material added since the last inventory submittal that exceeds 55 <br /> gallons,500 pounds,or 200 cubic feet at any one time must be reported. Also,any hazardous material deleted <br /> from your inventory since the last inventory submittal must also be reported. <br /> In order to comply with this years annual inventory submittal, please read and follow all of these steps: <br /> 1. Review the inventory that you submitted last year to the Office of Emergency Services and your <br /> current inventory of reportable hazardous materials. <br /> 2. Determine if there have been any changes as defined above. <br /> 3. Complete a Chemical Inventory Form(copies included in this packet) only for any added ma- <br /> terials, deleted materials, and/or change in quantity greater than 100 percent of a previously <br /> reported material. <br /> 4. In completing necessary Chemical Inventory Forms, please follow these guidelines. Indicate at <br /> the top of each chemical listing whether the chemical is an addition, deletion,or change of over <br /> 100 percent. For additions, complete all information requested for each added material. For <br /> deletions, only complete the "Chemical Name", "Trade Name", and "Daily Amount Stored" <br /> sections of the inventory report form for each deleted material. For changes of more than 100 <br /> percent, complete the "Chemical Name", "Trade Name", and "Daily Amount Stored" sections of <br /> a Chemical Inventory Form for each material which has changed quantity more than 100 percent. <br /> Show any other changes applicable for all changes submitted. <br /> 5. Submit Chemical Inventory Forms for any inventory changes with the completed Inventory Cert- <br /> ification Form which will constitute verification that all other materials listed on your previous <br /> report remain the same. <br /> 6. If there have been no changes in your inventory as defined above, simply complete and sign the <br /> Inventory Certification Form and return it to our office. _ <br /> NOTE: If you have changed, added or removed any storage locations for your hazardous materials, <br /> be sure to report them as part of your HMMP update. <br /> If this is the first inventory submittal by your business, then you must complete Chemical Inventory Forms <br /> for all hazardous materials handled by your business at or above 55 gallons, 500 pounds,or 200 cubic feet at <br /> any one time. <br /> Please return all completed forms along with the HMMP updates as outlined in the cover letter to this packet. <br />
