SAN JOAQUIN COUNT <br /> HAZARDOUS MATERIALS MANAGEMENT PLAN <br /> PLEASE READ ALL INSTRUCTIONS FOR EACH SECTION <br /> 1. Hazardous Materials Management Plans MUST be complete,accurate,ledgible and completed in <br /> ink or typed. <br /> 2. Original signature MUST be submitted and MUST be signed in ink. No copies or facsimile of <br /> signature will be accepted. <br /> 3. A plan shall be completed for each facility which has a different facility site address. <br /> 4. The attached forms or our format will be the only form accepted. Additional information or sections <br /> can be added by the business if desired. <br /> 5. At least one copy shall be retained by the business for its own use during an emergency. The HMMP <br /> shall be readily accessible for use by employees or inspection by County representatives. <br /> 6. All the areas in Section 2 at the bottom must be completed. This includes the Dun &Bradstreet <br /> Number by calling(215)882-7748 and the Standard Industrial Code(SIC)Number by calling your <br /> Planning, Building or Finance Department or visit the local library. <br /> 7. After your HMMP has been reviewed,your annual fee will then be generated from your inventory, <br /> with payment due within 30 days of receipt. All business licenses and building permits must be <br /> reviewed for compliance with the provisions of Chapter 6.95 of the Health and Safety Code. If a <br /> business is delinquent in their annual fee and/or their HMMP and inventory, it could lead to delays <br /> or rejection of the license or permit process. <br /> Please be aware that: <br /> Section 25514. (a)Any business that violates Sections 25503.5 to 25505,inclusive,or Sections 25508 to 25510,inclusive, <br /> the administering county or city in an amount of not more than two thousand doll ($2,000)for <br /> each day in which the violation occurs. If the violation results in,orsignificantly contributes to,an emergency,including <br /> afire,the business shall also be assessed the full cost of rhe county or city emergency response,as well as the cost of clean- <br /> ing up and disposing of the hazardous materials. <br /> (b) Any business that knowingly violates Sections 25503.5 to 25505, inclusive, or Sections 25508 to 25510, <br /> inclusive,after reasonable notice of the violation shall be civilly liable to the administering county or city in an amount <br /> not to exceed five thousand dollars($5,000)for each day in which the violation occurs. <br /> Section 25514.3. Any person that knowingly violates Sections 25503.5,25503.7,25505,25508,25509,25509.3,25510, <br /> or25533 after reasonable notice ofthe violation,is upon conviction,guilty ofa misdemeanor.
