Church St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I CLASS 11,or MINOR-Notice to Comply) <br /> ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br /> Class II violations: (20, 45, 49) <br /> 20. A 55 gallon drum of used oil and water was observed without a hazardous property listed. All <br /> hazardous waste containers shall be marked with the following information: <br /> ' the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, toxic, reactive, corrosive) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> The label was corrected at the time of inspection. Ensure that all hazardous waste containers are <br /> marked with all the required information. <br /> REPEAT VIOLATION <br /> 45. The contingency plan for this facility did not include the locations of emergency equipment at the <br /> facility. A contingency plan shall include all of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> V/ or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br /> & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br /> location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used <br /> -telephone number of Cal EMA <br /> This information must be kept current at all times. Immediately update the contingency plan for this <br /> facility to include all of the necessary information and submit a copy of the updated portions of the plan <br /> to the EHD by September 16, 2011. <br /> REPEAT VIOLATION <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY I UBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspect \ Received Qy: rifle: <br /> EHD 2302-00 7/12/11 CONTINUATION FORM <br />
