Church St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 49. The written training program and training records on site were incomplete. Facility personnel shall <br /> complete a training program that teaches them to perform their duties in accordance with Title 22 <br /> regulations. This program shall be directed by a person trained in hazardous waste management <br /> procedures and shall include the following: <br /> azardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> e training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> -written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Tffiffiediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a copy of the training program and records the the EHD by September 16, 2011. <br /> REPEAT VIOLATION <br /> Notice to Comply - Minor violations: (85) <br /> 85. Copies of universal waste disposal records were not found on site. A generator of universal waste <br /> / (batteries, mercury containing equipment, bulbs, electronic devices, CRTs, aerosol cans) shall keep a <br /> record of each shipment of universal waste on site for at least three years. The record may take the <br /> form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the following: <br /> - Name & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 ev 0T/ 11 CONTINUATION FORM <br />
