Identify the problem. <br /> � <br /> 2. Initiate business emergency plan. <br /> 3. Perform safety requirements <br /> 4. Phone emergency response agencies and provide: <br /> a. Nature of incident(spill,explosions, fire, etc.) <br /> b. Location of incident <br /> c. Time/date incident occurred <br /> d. Name of hazardous material(s) <br /> e. Approximate quantity involved. <br /> f. Safe location on or near facility where emergency responders can meet <br /> company Emergency Coordinator <br /> g. Number of injuries and cause of injuries <br /> h. Whether facility is evacuated <br /> i. Whether surrounding area should be evacuated <br /> j. Whether individuals have been contaminated <br /> k. Write down State Control case number and National Response case <br /> number. <br /> 1. Record time of call and the name of contact if possible. <br /> ' r - <br />
