A generator shall keep a record of <br />any test results, waste analyses, or other determinations made in accordance with hazardous waste regulations <br />for at least three years from the date that the waste was last sent to on-site or off-site treatment, storage, or <br />disposal. A copy of the most recent waste analysis is attached and available at the facility. <br />Concern #110: The contingency plan for this facility did not include the location of emergency equipment at <br />the facility, the evacuation map, or the phone number for CalEMA. A copy of an updated contingency plan <br />with the necessary information is attached and available this facility. <br />Concern #117: The training records on site were incomplete. The training records do not include a description <br />of the training to be given to each employee or a record that every employee received the training. The titles <br />used in the job descriptions on site did not completely match the job titles used in the training sign in sheets. <br />Please find attached a revised training log including employee job descriptions that match employee job <br />titles. All of these training topics are completed at least once annually and within the first 6 months of hire. <br />(See attached updated training log). Facility will document any training absentees on log and train upon <br />arrival. <br />Concern #119: Manifest numbers 009574416JJK (5/1/12), 009574415JJK (5/1/12), and 009561607JJK <br />(1/17112) were sent under the EPA ID number and generator & site address for the Sacramento facility. <br />Created and mailed notifications with all necessary corrections to DTSC, all wastes were properly identified <br />under the correct EPA ID number and address. Attached are notification letters. <br />Concern #121: Manifest numbers 006069742JJK (3/1/10), 007083765JJK (4 /23/10), 008481617JJK (7/18/11) <br />and 008979032JJK (7 /19/11) were found on site at the facility but were not on record with the Department of <br />Toxic Substances Control (DTSC). Within 30 days of each shipment, a generator shall submit a legible copy of <br />each manifest used to DTSC. Copies of the aforementioned manifests were submitted to the DTSC on <br />(10115112). Attached is a copy of manifests and envelopes. <br />Concerns #122,123 &124: Manifest numbers 008816929JJK (9/30/11) and 009740395JJK (3/16/12) were not <br />found on site.
