Clark Dr., Stockton <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />110. (continued) <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br />soil, or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, contractors, and <br />state & local emergency response teams <br />-names, addresses & phone numbers (office & home) of the Emergency Coordinator <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br />location, physical description, and a brief outline of its capabilities <br />-evacuation plan, including routes and signals to be used <br />-telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />October 26, 2012. <br />REPEAT VIOLATION <br />117. The training records on site were incomplete. The training records do not include a description of <br />the training to be given to each employee or a record that every employee received the training. The <br />titles used in the job descriptions on site did not completely match the job titles used in the training sign <br />in sheets. Facility personnel shall complete a training program that teaches them to perform their duties <br />in a way that ensures the facility's compliance with Title 22 regulations. This program shall be directed <br />by a person trained in hazardous waste management procedures. The training program shall be <br />completed by each employee within six months of hire, prior to unsupervised work, and shall be <br />reviewed annually. Training records shall be maintained on site until closure of the facility and shall <br />include: <br />- name & job title of each employee <br />- written job description for each position Qob title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately update the training records to contain all of the required information and ensure that all <br />employees complete this training within six months of hire and at least annually thereafter.
