(continued) <br />- Name & address of destination facility <br />- Quantity & type of universal waste <br />- Date of departure/shipment <br />Begin this practice immediately. <br />REPEAT VIOLATION <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by October 25, 2012 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />Notes: (corrected on site) <br />- A partially full aerosol can of marking paint was observed in the trash can in the shop. No person shall <br />knowingly, or with reckless disregard for the risk, treat, handle, transport, dispose, or store any <br />hazardous waste in a manner which causes an unreasonable risk of fire, explosion, serious injury, or <br />death. The aerosol can was removed from the trash can for proper disposal at the time of inspection. <br />- An open cardboard box of used fluorescent bulbs was observed in the office. Universal waste lamps <br />shall be contained in a closed container or package that is structurally sound, adequate to prevent <br />breakage, and compatible with the contents of the lamp.
