The contingency plan for this facility did not include the location of emergency equipment at the <br /> facility, the name, address, and phone number of the facility Emergency Coordinator and alternate, <br /> arrangements made with local agencies, the evacuation plan, or the phone number for CaIEMA. A <br /> contingency plan shall include all of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br /> & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br /> location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used <br /> -telephone number of Cal EMA <br /> This information must be kept current at all times. Immediately update the contingency plan for this <br /> facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br /> September 24, 2011. <br /> REPEAT VIOLATION <br /> 49. A training records on site were incomplete. The training records do not include written job <br /> descriptions or a description of the training to be given to each employee. The job titles were <br /> inconsistently listed with the employee names on the sign-in sheets. At the time of inspection, <br /> documentation could not be provided to indicate that all employees had completed the complete annual <br /> refresher training. Facility personnel shall complete a training program that teaches them to perform <br /> their duties in a way that ensures the facility's compliance with Title 22 regulations. This program shall <br /> be directed by a person trained in hazardous waste management procedures and shall include the <br /> following: <br /> - hazardous waste management procedures relevant to the positions in which they are employed, <br /> including contingency plan implementation <br /> -emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> -communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> -shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually.
