Used fluorescent bulbs were observed in an open box and uncontainerized without labels in the shop storage building. A generator shall label or mark universal waste with the words 'Universal Waste' and the type of waste (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol cans). Immediately label these wastes and ensure that all universal wastes are labeled with the required information. 84. Facility was unable to demonstrate the amount of time the used fluorescent bulbs were stored on site. According to Mr. Estevez, they may have been left over from the previous owner. A generator of universal waste(alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans) shall accumulate universal waste for no longer than one year from the date the waste was generated. Immediately dispose of this waste as universal waste and submit a copy of the disposal record to the EHD by February 20, 2011. Complete and submit a copy of the Return to Compliance Certification to the EHD by September 24, 2011 along with a statement documenting the corrective actions that have been or will be taken for each violation and any supporting paperwork. "This facility opened in January 2011. Note: (corrected on site) An emergency coordinator and modified contingency plan information was lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. The modified contingency plan was completed and posted at the time of inspection.
