No. CA2890090002 <br />Dear Mr. Cacapit: <br />July 8, 2014 <br />RECEIVED <br />JUL 14 2014 <br />ENVIRONMENTAL HEALTH <br />PERMIT/SERVICES <br />Per your request, please find attached the Uniform Hazardous Waste Manifests and <br />accompanying Land Disposal Restriction (LDR) forms (Attachments 1, 2, 3) that document the <br />transport and disposal of decontamination waste from the acid incident. The wastes were <br />disposed of at the Clean Harbors, San Jose facility and the Lawrence Livermore National <br />Laboratory (LLNL) Livermore Site, Decontamination and Waste Disposal Facility (DWTF). <br />The decontamination wastes are highlighted in yellow on each manifest. The manifest <br />information is summarized below. <br />Manifest Date Manifest # Line # Waste Description <br />3/24/13 00587090 JJK 3 Sulfuric acid liquid corrosive waste. <br />Inorganic solid decontamination waste contaminated with <br />D008 lead. <br />23 Inorganic solid decontamination waste contaminated with <br />sulfuric acid. <br />6/6/13 005865488 JJK 48 Inorganic solid decontamination waste contaminated with <br />sulfuric acid. <br />1/7/14 005870075 JJK 1 Aqueous rinsewater solution contaminated with D006 <br />cadmium. Approximately 800 gallons of acid <br />decontaminated rinsewater was bulked with 2500 gallons of <br />other Site 300 wastewater, totaling 3300 gallons. <br />The incident occurred as a result of an unplanned exothermic reaction during a chemistry <br />synthesis operation. The reaction released sulfuric acid to the floor and equipment inside the <br />room.
