I called and left a message for Scott Wilson with <br />LLNL and Alejandro Galdamez with DTSC that they have the wrong mailing <br />address for our department (was mailed to Eleanor Ratcliffe, Environ Health <br />Specialist, San Joaquin Public Health Services, 205 W-9the St, Tracy, CA <br />95376).l also followed up with an email. Printed and filed the email with the <br />correspondence in the HW file. <br />09/30/2015 Conducted a HW inspection. Only reviewed the disposal records, training <br />6 min records, and contingency plan. Walk through inspection has not be <br />conducted yet. Will return tomorrow, 10/1 to do walk through inspection. <br />Violations found: 1)HW determination 2) contingency plan incomplete 3) <br />illegal disposal 4) HW treatment without permit 5) minimize the release of <br />HW 6) failed to maintain all communication and decontamination <br />equipments 7) HW in a good container 8) closed containers 9)incompatible <br />wastes 10) empty containers 11) satellite accumulation exceeded 55 <br />gallons 12) stored satellite accumulation on site longer than 90 days 13) <br />HW labels 14) drained used oil filters 15) UW lamps 16) CRTs 17) UW <br />labels 18) UW stored onsite greater than 1 year 19) records of UW <br />shipment <br />09/30/2015 Prepped file for HW inspection.
