Responded to their request PR0514115 <br />240 min with EHD's decision. Provided a spreadsheet answering each request. PE 2249 SC 002 <br />Emailed to Thomas Kato, Environment, Safety and Health (ESH) Manager <br />and Dr. Frances Alston, ESH Director. <br />02/10/2016 Received call from Sav Macieri of LLNL stating that I failed to include a PR0514115 <br />60 min response to violation #715 in my spreadsheet. I acknowledged the mistake PE 2249 SC 002 <br />and told him I would modify the response ASAP. <br />I amended the spreadsheet to include violation #715 and emailed it to <br />Thomas Kato and Sav Mancieri. <br />02/11/2016 Received an email from Sav Mancieri stating I confused violation #202 (b) PR0514115 <br />30 min with violation #301 and reported it as "failed to minimize" violation instead of PE 2249 SC 002 <br />"disposed or caused disposal at an unauthorized point". After several email <br />exchanges I spoke with Sav on the phone and determined he meant to <br />reference violation #202(d), not (b), and I did include it under the wrong <br />violation in my response.
