This information was required to be submitted by April 27, 2012. This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. The following violations are <br /> outstanding violations from March 27, 2012: <br /> 101: Managed a hazardous waste without a EPA number <br /> 122: Failed to keep signed copy of manifest for 3 years <br /> 125: Failed to keep a copy of consolidated manifest for 3 years <br /> 127: Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years <br /> 128: Failed to retain copy of bill of lading for used oil filters for 3 years <br /> 301: Facility not maintained to minimize the release of a hazardous waste <br /> 404: Failed to keep containers of HW closed except when adding or removing HW <br /> 605: Failed to complete label containers or tanks of hazardous waste <br /> 610: Failed to store used oil filters in a closed, rainproof container that is labeled "drained used oil filters"and marked <br /> with the accumulation start date <br /> 812: Failed to keep records of each shipment of universal waste <br /> This is a Class II violation. <br /> 117 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Dark stains observed near the used oil tank, secondary containment of waste antifreeze and the engine part sitting on <br /> top of a pallet. Asked the general manager, Bao Nguyen, about the cause of spill near the used oil tank and he stated <br /> that it was from the oil pickup and not from a leak. Facilities shall be maintained and operated to minimize the <br /> possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human <br /> health or the environment. Immediately clean the spill, and manage according to Title 22 hazardous waste <br /> regulations.
