I. Notify the local authorities that evacuation of local areas may be necessary. <br /> 2. Notify the Local Emergency Planning Committee,report your name and phone number,name and address <br /> of your facility,time and type of incident,name and quantity of materials involved the extent of any <br /> injuries and possible hazards to health or the environment. <br /> F. During the emergency,the Incident Commander(IC),must take all reasonable steps necessary to assure that any other <br /> fires,explosions,or releases of hazardous substances do not occur or spread. These steps include halting production, <br /> removing or isolating all hazardous waste containers,and cleaning up any spills. The IC is also responsible for the <br /> removal of any hazardous waste that may need to be hauled away. If there are any questions,consult with QA <br /> Manager. <br /> G. The IC is obligated to monitor any other possible problems(i.e.pressure build up leaks,exploding gas cylinders,etc.) <br /> caused by the emergency. <br /> H. Immediately following any emergency,you must provide for complete clean up measures as necessary including any <br /> treatment,storage,or disposal of recovered waste,contaminated soil, or waters. No incompatible wastes may be <br /> treated. If there are questions,consult with the QA Manager. <br /> 1. Reporting Requirements: <br /> I. Following clean-up and before resuming production,you must notify the fire department that your facility <br /> is completely cleaned up,in accordance with your contingency plan,and ready to resume operations. <br /> 2. As soon as practical, a written follow-up report is to be sent to the State Emergency Response Commission <br /> and to the Local Emergency Planning Committee. It should contain more specific data on the release. <br /> a) Response and containment procedures. <br /> b) Any known or anticipated acute or chronic health risk <br /> c) Where appropriate, advice regarding if medical attention is necessary for the exposed <br /> individuals. <br /> 3. Notify Environmental Compliance Specialist-Michael Stark*,(530)886-0664. <br /> IX. Building Site Maps See attached maps. <br /> X. Clean Up Procedures(Spill Response Plan) <br /> A. Following equipment,materials and supplies are available at the facility to expeditiously control,contain and remove <br /> spilled material: <br /> Emergency Equipment Description <br /> There is a safety equipment and clean-up locker located in the area directly outside the QC office. <br /> This locker contains the following: <br /> I Pail with wringer 3 Sets vinyl protective suits <br /> 1 or 2 mops 3 Pairs rubber boots <br /> 2 Pairs of goggles I Container of absorbent <br /> 1 Roll of hazard tape 2 Pairs of rubber gloves <br /> I Plastic shovel <br /> A spill control cabinet is located near the receiving dock in the warehouse. <br /> This cabinet contains the following items: <br /> I Roll of hazard tape 6 pair of rubber boots <br /> 1 bag of absorbent 1 box of nitrile gloves <br /> Emergency Action Plan 7 <br /> W98 Last Update:02/07/00 <br />
