Failed to perform annual hazardous waste training for employees. The last documented <br /> "Hazardous Material Emergency Response, Spill Prevention, Mitigation & Contingency Plan"was last <br /> performed on 06/26/12. Per Facility Mgr, the training was scheduled for June 2013 but was reschedule <br /> to July 2013 due to trainer unavailability. Training shall be provided by a person trained in hazardous <br /> waste management procedures and shall include instructions which teaches facility personnel <br /> hazardous waste management procedures, including contingency plan implementation relevant to <br /> positions in which they are employed. At minimum, training shall include emergency procedures, <br /> equipment, and system. Each personnel shall be trained within 6 months of hire and provided an annual <br /> refresher. Correct by providing hazardous waste training records to EHD within 30 days. <br /> Submit to SJC EHD within 30 days (08/15/13): <br /> 1. one copy of the completed 'Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. requested /supporting documentation ie. training records, disposal receipt for Zep <br /> Notes: <br /> - In 2012 year, facility generated approx. 6526 gallons (26.1 Tons) of used oil, 2025 gallons (8.1 Tons) of <br /> used antifreeze, and 1.54 tons of waste paint/ paint filters/ oily sludge = 36.7 Tons of haz waste. <br /> - Satellite accumulation (point of accumulation) HSC 25200.3.1/ CCR 66262.34(e) - up to 55 gallons per <br /> waste stream, at point of generation, under control of operator. on site up to 1 yr or 90 days from date <br /> container became full. Must be moved to Haz Waste Storage area within 3 days it became full. Storage <br /> option for Body Shop 30 gallon drum of waste paint. Per Body Shop mgr, generates <10 gal in 6 months. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE OVE 40TED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTI A A TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
