There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Eliseo Nunez and Lou Ball have been appointed as the emergency coordinators and this information is now posted by the facility phone. Continue to use the provided form and ensure that the required information remains posted by the facility phone. 79. Approximately 14 universal waste(UW) lamps were left in the maintenance shop not in a closed container. UW lamps must be stored in a closed container and in a manner that prevents breakage and release to the environment. Lou Ball and Eliseo Nunez had their employees transfer the UW lamps into a closed container and properly labeled as UW. Continue to ensure all UW containers are closed when not adding or removing waste. 83. Approximately 14 universal waste(UW) lamps were left in the maintenance shop and not labeled at all. UW must be labeled to identify the type of UW. Lou Ball and Eliseo Nunez had their employees label the UW lamps as universal waste lamps. Continue to ensure all UW is labeled to identify the type of UW. Department of Toxic Substances Control UW fact sheet was provided to Lou Ball and Eliseo Nunez. 84. Approximately 14 universal waste(UW) lamps were left in the maintenance shop and not marked with an accumulation start date. The facility must be able to demonstrate the length of time the UW has been accumulated from the date it became a waste. Lou Ball and Eliseo Nunez had their employees mark the UW lamps with the accumulation start date. Continue to ensure all UW is marked with an accumulation start date and properly disposed of within one year of the accumulation start date. Hazardous Waste(HW) labels, HW label example, and a copy of the San Joaquin County Small Business Hazardous Waste disposal program brochure was provided. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE (x122).
