An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Eliseo Nunez and Lou Ball <br />have been appointed as the emergency coordinators and this information is now posted by the facility <br />phone. Continue to use the provided form and ensure that the required information remains posted by <br />the facility phone. <br />79. Approximately 14 universal waste(UW) lamps were left in the maintenance shop not in a closed <br />container. UW lamps must be stored in a closed container and in a manner that prevents breakage and <br />release to the environment. Lou Ball and Eliseo Nunez had their employees transfer the UW lamps into <br />a closed container and properly labeled as UW. Continue to ensure all UW containers are closed when <br />not adding or removing waste. <br />83. Approximately 14 universal waste(UW) lamps were left in the maintenance shop and not labeled at <br />all. UW must be labeled to identify the type of UW. Lou Ball and Eliseo Nunez had their employees label <br />the UW lamps as universal waste lamps. Continue to ensure all UW is labeled to identify the type of UW. <br />Department of Toxic Substances Control UW fact sheet was provided to Lou Ball and Eliseo Nunez. <br />84. Approximately 14 universal waste(UW) lamps were left in the maintenance shop and not marked <br />with an accumulation start date. The facility must be able to demonstrate the length of time the UW has <br />been accumulated from the date it became a waste. Lou Ball and Eliseo Nunez had their employees <br />mark the UW lamps with the accumulation start date.
