Harney Ln. Lodi, CA 95240 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. Since <br />these containers have been on site longer than 180 days, Immediately contact a licensed hazardous <br />waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by <br />2-4-11. Minor -Notice to Comply. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 2-4-11. <br />Note: The following were corrected on site. <br />20. The two containers (5 & 3 gallons) of used oil next to the the used oil tank, the 30, 15, & 15 gallon <br />drums of oily diesel, and an open container of waste ballasts that was left outside the shop were not <br />labeled at all. All hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Lou Ball and Eliseo Nunez had their employees pour the used oil from the two containers (5 & 3 gallons) <br />into the used oil tank. and properly labeled the 30. 15, & 15 gallon drums of oily diesel and the new <br />container of ballasts with the labels provided. Continue to ensure that all hazardous waste containers <br />are marked with all the required information. <br />23. Drained used oil filters were left to accumulate in a five gallon bucket near the waste oil tank. A used <br />coil filter was left on top of another 5 gallon bucket near the waste oil tank. Used oil filters must be stored <br />in a closed, rainproof container, labeled as "drained used oil filters," and marked with an accumulation <br />start date. Lou Ball and Eliseo Nunez had their employees transfer the drained used oil filters into a <br />closed container. labeled as "drained used oil filters." and marked with an accumulation start date. <br />Continue to ensure that all used oil filters are managed in this way <br />27. An open container of waste ballasts was left outside the shop. All hazardous waste containers shall <br />be closed at all times except when adding or removing waste. Lou Ball and Eliseo Nunez had their <br />employees transfer the waste ballasts into a closed container and properly labeled as hazardous waste. <br />Continue to ensure all hazardous waste containers are closed when not adding or removing waste. <br />ALL EHII STAFF TIME ASSOCIATED WITH FAILING TOCOMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT NY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received <br />Title:.* <br />EHD 23-02-003 Rev 08/10/10 //�/�/ CONTINUATION FORM <br />
