The procedures (Bravo Conversion Frame Installation Instructions, <br />dated 3/25/08) submitted in the approval process must be followed <br />in their entirety. Any variations in the installation procedures must <br />be reviewed and approved by the State Water Board. <br />Per OCR, Section 2636(g)(3)(C), the Underground Storage Tank <br />Program Manager may modify or revoke this approval, at any time, <br />if there is any evidence that CONV-B2000 fails to adequately <br />protect the soil and ground water from unauthorized release. <br />Each frame is only considered to be approved as long as it is <br />manufactured, installed, operated, and maintained in accordance <br />with bravo's requirements and the State Water Board's regulations. <br />Corrective Action: Immediately, take appropriate action by contacting the <br />manufacturer and the State Water resource control Board to correct the <br />violations. <br />If the conversion frames repair/ replacement can't be performed immediately, <br />there is a possibility that a Notice of Significant Violation will be issued, which <br />could lead to Red Tagging the Diesel UST system. <br />Please be advised that failure to correct the violation will cause <br />continuing/additional violations and may result in further legal action. <br />If you have any questions, please contact Muniappa Naidu, REHS, at (209) 468- <br />3439. <br />Muniappa Naidu, Senior RENS <br />Underground Storage Tank Program <br />CC.
