Lindsay St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> This is a routine Hazardous Waste (HW) inspection. The initial inspection occurred on 0672angd <br /> completed on 07/07/2010. The following Items #49,79,83.84 are Repeat Class II violatio#7,32,45,53,54 are minor violations Notice to Comply:7. It has come to the attention of this office (EHD) during this inspection that the sludge f <br /> clarifier connected to POTW, and the liquid and the sludge from the bus wash, is being managed as <br /> non-RCRA waste. EHD does not have any information supporting the waste is not HW. Also observed <br /> two wings of waste paint booth filters in the storage area north of the service bay which the generator <br /> stated are to be disposed. <br /> Generator shall make a HW determination of the waste and manage it accordingly. <br /> ` Make a HW determination of the above waste and manage it accordingly, or manage them as HW in <br /> accordance to Title 22 If the waste is determined to be not HW, submit evidence in support of that <br /> determination. Submit HW determination and supporting document (including HW disposal receipts if <br /> applicable) to EHD by August 8, 2010. <br /> 32. Observed one approximately 300-gallon used anti-freeze tank with no secondary containment in the <br /> bus wash area. The tank was installed after 1991 and has not been assessed according to generator. <br /> -- A new HW tank shall be assessed by an independent State-certified professional engineer (PE), and <br /> reassessed every five years if there is no secondary containment. <br /> Submit a copy of the tank assessment in accordance to CCR 66265-191 by a PE to EHD by August <br /> 8, 2010. A guidelinejor Tank Assessment and what's required in the assessment has been provided to <br /> generator on site. /.4 A (L <br /> 45. Contingency Plan incomplete: The HMMP and SPCC on site have not incorporated the HW <br /> management provisions. <br /> -- The HW Contingency Plan shall (a) describe the action facility personnel shall take in response to <br /> fires, explosions, or any unplanned sudden or non-sudden release of HW at the facility, (b) <br /> Arrangements agreed to by local police, fire, hospital, contractors, State and local emergency response, <br /> (c) List of names, addresses, and phone numbers of all qualified emergency coordinators, (d) List of all <br /> emergency equipment at the facility, (e) evacuation plan, (f) current phone number of State Office of <br /> Emergency Services. <br /> Submit a copy of the HW Contingency Plan to EHD by August 8, 2010.
