(Repeat) Current personnel training records were not available for inspection. <br /> - Facility shall retain on site a copy of the HW management training records of current employees for <br /> at least 3 years of hiring. The record shall include. (a) Job title of each position related to HW <br /> management, (b) written description of each of these positions including requisite skill and qualification, <br /> (c) written description of the type and amount of training provided for the positions, (d) and <br /> documentation of training completed by the employee filling the position. <br /> ' Submit a copy of the current personnel training records to EHD by August 8, 2010. <br /> 53. No report of the changes in volume and toxicity of HW according to generator. <br /> - Generator shall submit a report to the department (DTSC) of the changes in volume and toxicity of <br /> waste achieved through waste reduction every two years. <br /> Submit a copy of the report of the changes in volume and toxicity achieved through waste reduction, <br /> and continue to the department every two years <br /> 54. No summary progress report of the result of implementing source reduction according to generator. <br /> - Generator shall submit to the department every four years a summary progress report that briefly <br /> summarizes the results of implementing the source reduction methods for each waste stream over the <br /> previous four years, and the estimate reduction the next review and plan. <br /> Submit to the department a summary progress report every four years. <br /> 79,83,84. (Repeat) Observed 20 waste fluorescent lamps (lamps) at the upstairs storage not in <br /> container and not labelled. Unable to determine the length of storage <br /> - Universal Waste (UW) lamps shall be stored in a container that prevents breakage and release to the <br /> environment, label to identify the type of UW, be able to demonstrate the length of storage, and <br /> managed within one year of storage. <br /> ' Immediately properly dispose the above UW-lamps and submit a copy of the disposal receipt to EHD <br /> by August 8, 2010. Begin storing all UW-lamps (and other UW) in a manner that prevents breakage and <br /> release to the environment, label to identify the type of UW, and manage within one year of storage. <br /> NOTES <br /> -The facility has generated 40,305 pounds of HW, or 20.15 tons in 2009.
