Pressure was <br /> applied for five minutes. The pneumatic packer was removed from the well casing and the MW <br /> was topped of with neat cement. After the MW was pressure grouted with neat cement, a cutting <br /> torch was used to cut the well casing and the conductor casing 1.5-feet bls. The existing asphalt <br /> road was cut 1-foot larger in diameter than the original borehole diameter. The MW vault, <br /> asphalt and road base were removed to approximately 1.5-feet bls. Neat cement was poured on <br /> top of the pressure grouted MW to form a 6-inch thick mushroom cap. Road base from the <br /> original excavation was used to back fill the excavation to 6-inches bls. On 07-Apr-06, a <br /> temporary 6-inch thick cold asphalt patch was placed and compacted to land surface in the <br /> excavation. On 14-Apr-06 Bonadonnas Asphalt Repairs, Inc. from Stockton, CA, removed the <br /> temporary cold asphalt patch and placed and compacted hot paving asphalt to land surface. The <br /> County of San Joaquin Department of Public Works inspected and approved the asphalt patch. <br /> Destruction of Groundwater Monitoring Well KI-7 <br /> On 06-Apr-06 WDC attempted to pull the submersible pump from MW KI-7. When the pump <br /> was pulled up to 9-feet bls,the pump became wedged within the well casing. The well casing at <br /> 9-feet bls transitions from 4-inch PVC SCH 40 to 4-inch mild steel (Appendix A). Within the <br /> well casing, a small lip approximately 1/16 of an inch was located at the transition from PVC to <br /> mild steel. After investigation the sounding tube wedged between the pump and the well casing. <br /> WDC pulled on the pump drop pipe until the PVC drop pipe snapped at the top of the pump. <br /> WDC removed the remaining pump wiring and sounding tube from the well casing by <br /> manufacturing a fishing tool on site. WDC was not able to remove the pump from the well <br /> casing. DUDEK requested approval from the San Joaquin County Environmental Health <br /> Department(SJCEHD) and the California Department of Toxic Services (DTSC)to push the <br /> pump to the bottom of the MW and pressure grout the pump in place. Harlen Knoll from <br /> SJCEHD was on site and observed the lodged pump and approved the variance of the MW <br /> destruction. Glenna McMahon with DUDEK received notification of approved variance in MW <br /> destruction from Kate Berger with the DTSC on 06-Apr-06 at 11:10 AM. WDC pushed the <br />
