MONITORING AND REPORT14PROGRAM NO. 5-01-XXX • -5-
KEARNEY-NATIONAL,INC.AND UPRIGHT CONSTRUCTION
FORMER KEARNEY-KPF FACILITY
GROUNDWATER TREATMENT AND DISPOSAL SYSTEM
SAN JOAQUIN COUNTY
4. A comparison of monitoring data to the effluent limits and discharge specifications and an
explanation of any violation of those requirements;
5. A description of the groundwater sampling event, including field logs. At a minimum, field logs
shall contain water quality parameters measured before, during, and after purging, method of
purging, depth of water, volume of water purged, etc.;
6. Copies of all laboratory analytical report(s);
7. A calibration log verifying weekly calibration of any field monitoring instruments (e.g., DO, pH,
EC meters)used to obtain data;
8. Groundwater contour maps for all groundwater zones;
9. Isocontour contaminant concentration maps for all groundwater zones;
10. A table showing well construction details such as well number, groundwater zone being
monitored, coordinates (northings and eastings), ground surface elevation, reference elevation,
elevation of screen, elevation of bentonite, elevation of filter pack, and elevation of well bottom;
11. A table showing historical lateral and vertical (if applicable) flow directions and gradients; and
12. Cumulative data tables containing the water quality analytical results and depth to groundwater.
C. Annual Report ►""
An annual report shall be submitted to the Regional Board by 1 February of each year. This report j
shall contain an evaluation of the effectiveness and progress of the remediation, and may be submitted
with the fourth quarter monitoring report. The annual report shall contain the following minimum
information: T
1. Tabular and graphical summaries of all data collected during the previous year;
2. Groundwater contour maps and contaminant concentration maps containing all data obtained
during the previous year;
3. Data for monitoring and analysis performed on an annual basis; V
4. A discussion of the long-term trends in the concentrations of the pollutants in the groundwater i
monitoring wells;
5. An evaluation of the performance of the groundwater treatment system, including a description of
all remedial activities conducted during the year, an analysis of their effectiveness in removing the
contaminants and whether the contaminant plume is being captured by the extraction system or is 1
continuing to spread, as well as a forecast of the flows anticipated in the next year;
