Libbey Owens Ford Company shall be responsible for a self <br /> monitoring program and shall submit a monthly report to the City <br /> of Manteca, due on or before the 10th day of each month, <br /> containing the following information: <br /> a. Date and time each batch discharge began and ended. <br /> b. Total discharge for the month in gallons. Cumulative <br /> discharge in gallons. <br /> C. Report (s) in accordance with Condition 8 above. <br /> d. Report in accordance with Condition 9 above. <br /> Submit Reports to: City of Manteca <br /> 1001 W. Notify the City of Manteca Wastewater Quality Control <br /> Facility by calling Mr. Warren Shannon, Wastewater Systems <br /> Superintendent, at (209) 239-8433 when discharges begin and end. <br /> 12. Failure to comply with City of Manteca Ordinance No.644, <br /> failure to submit reports, or failure to notify the Wastewater <br /> Quality Control Facility when the discharce begins and ends will <br /> result in suspension or termination of this permit. <br /> Michael F.
