0 0 <br />9 -6- <br />f. Personnel shall dispose of all disposable PPE immediately <br />after removal of such items. Under no circumstances shall <br />disposable PPE items be reused. <br />g. All personnel shall be trained in the site-specific <br />emergency procedures, including the location of emergency <br />telephone numbers and hospital route maps. <br />h. Equipment, including heavy equipment, shall be kept in <br />proper working order and shall be kept free of accumulate <br />lubricants, contaminants or other hazardous or flammable <br />substances. <br />i. No containers of fuels or other flammables will be kept <br />within 100 feet of any drilling or sampling operation. <br />j. No person shall enter an open excavation or confined space. <br />k. All policies promulgated in this health and safety plan <br />• shall be followed by all employees. Changes in any <br />procedures or policies contained in this plan shall only be <br />implemented after approval from the Lambert Construction/ <br />Environmental Health and Safety Officer has been obtained. <br />11 <br />
