The training observed on site <br />was lacking: <br />-written job description for each position <br />-written description of the type and amount of both introductory and continuing training that will be given to each <br />person filing positions listed above; <br />-records that document that the training has been given to and completed by facility personnel. <br />Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br />employee training is properly documented. <br />This is a repeat violation, Class II. <br />120 <br />HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br />The consolidated manifest receipts for: <br />-January 18, 2018 for used oil <br />-June 3, 2016 for used oil <br />were missing the driver's signature. All copies of each receipt shall contain all of the following information: <br />- The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative; <br />- The date of the shipment; <br />- The manifest number; <br />- The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br />type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if applicable; <br />- The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported; <br />- The transporter's name, address, and identification number; <br />- The driver's signature; <br />- A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br />all required information. <br />This is a minor violation. <br />FA0013605 PR0517826 SCO01 07/26/2018 <br />EHD 22-02 Rev. 10/31/2017 Page 7 of 10 Large Quantity Hazardous Waste Generator OIR <br />1868 E.
