The facility's contingency plan is incomplete. The primary emergency coordinator was listed as Mike Botelho. Mike Botelho is no longer employed at this site per parts manager. The contingency plan must include: 1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non -sudden release of hazardous waste to air, soil or surface water at the facility; 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and State and locai emergency response teams to coordinate emergency services; 3. List of names, addresses, and phone numbers (officelhomelcell) of all persons qualified to act as emergency coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in which they will assume responsibilities, 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; 5. Evacuation routes for facility personnel where there is a possibility of evacuation; 6. Current telephone number for State Office of Emergency Services. If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. This is a repeat violation, Class Il. 117 CCR 66265.16{a -c} Failed to complete training on hazardous waste management and emergency response procedures. No training records were available for review during the inspection. Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual review of the initial training received. Ala minimum, the training program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where applicable: 1procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; 2. key parameters for automatic waste feed cut-off systems; 3. communications or alarm systems; 4. response to fires or explosions; 5, response to ground -water contamination incidents; and 5. shutdown of operations. The owner or operator shall maintain the following documents and records at the facility, 1. the job title for each position at the facility related to hazardous waste management, and the name of each employee filling each job; 2. written job description for each position listed above; 3. written description of the type and amount of both introductory and continuing training that will be given to each person filing positions listed above; 4. records that document that the training has been given to and completed by facility personnel. Training records on current employees shall be kept until closure of the facility. Training records on former employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately provide employees with initial or refresher hazardous waste training. Submit a copy of the roster and the syllabus to the EHL). This is a Class It violation.
