The primary emergency coordinator was listed as Mike Botelho. Mike <br />Botelho is no longer employed at this site per parts manager. The contingency plan must include: <br />1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br />non -sudden release of hazardous waste to air, soil or surface water at the facility; <br />2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br />State and locai emergency response teams to coordinate emergency services; <br />3. List of names, addresses, and phone numbers (officelhomelcell) of all persons qualified to act as emergency <br />coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br />all others in order in which they will assume responsibilities, <br />4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br />5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br />6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br />plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br />are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br />fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br />This is a repeat violation, Class Il. <br />117 <br />CCR 66265.16{a -c} Failed to complete training on hazardous waste management and emergency response <br />procedures. <br />No training records were available for review during the inspection. Personnel who handle hazardous waste shall <br />be trained within six months of assignment and take part in an annual review of the initial training received. Ala <br />minimum, the training program shall be designed to ensure that facility personnel are able to respond effectively to <br />emergencies by familiarizing them with emergency procedures, emergency equipment, and emergency systems, <br />including where applicable: <br />1procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br />2. key parameters for automatic waste feed cut-off systems; <br />3. communications or alarm systems; <br />4. response to fires or explosions; <br />5, response to ground -water contamination incidents; and <br />5. shutdown of operations. <br />The owner or operator shall maintain the following documents and records at the facility, <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. Immediately <br />provide employees with initial or refresher hazardous waste training.
