LFR, Inc. <br /> by using safe handling practices, proper lifting techniques, and proper personal safety <br /> equipment such as steel-toed boots and sturdy work gloves. Where practical, <br /> mechanical devices will be utilized to assist in the movement of equipment and <br /> materials. Workers will not attempt to move heavy objects by themselves without using <br /> appropriate mechanical aids such as drum dollies or hydraulic lift gates. <br /> 6.10 Confined Space Entry <br /> Entry into confined spaces will be conducted in strict accordance with 8 CCR 5157. <br /> Before workers may enter a permit-required confined space, an entry permit must be <br /> completed and approved by the Director of Health and Safety and all requirements for <br /> entry must be met. Confined spaces may be described as having, but not being limited <br /> to, the following characteristics: <br /> • is large enough to permit an employee to enter and perform work <br /> • has limited or restricted means of entry and exit <br /> • is not equipped, designed, or intended for continuous human occupancy <br /> In addition, one or more of the following conditions may be present in a permit- <br /> required confined space: <br /> • contains or has the potential to contain a hazardous atmosphere <br /> • contains or has the potential to contain a material with the potential to engulf or <br /> entrap an employee <br /> is so configured that an employee may become trapped, disoriented, or asphyxiated <br /> by wall configurations or smaller cross sections <br /> contains any other established safety or health hazards, such as energized <br /> equipment or moving parts <br /> All fluid, electrical, and steam lines and other sources of energy into confined spaces <br /> must be completely isolated before entry. The following conditions must be met before <br /> entry is permissible (air monitoring may be necessary to verify these conditions): <br /> • Flammable vapor must be at a concentration less than 10 percent of the lower <br /> explosive limit (LEL). <br /> • Oxygen must be at a concentration greater than 19.5 percent and less than 23.5 <br /> percent. <br /> • Toxic substances must be at a concentration less than their respective permissible <br /> exposure limits. <br /> In addition, the following roles must be designated before entry into permit-required <br /> confined spaces is allowed: Entry Supervisor; Attendant; and Authorized Entrant(s). <br /> Page 12 KMEP-SMkwn-HASPv1.doc:LFR <br />
