Y�^ <br />3131 Arch Road, Stockton <br />Page 2 <br />On July 8, 1998, an unauthorized release form was completed for the MTBE and <br />TPH-d contamination associated with the 12,000 gallon USTs. <br />On July 21, 1999, the 1000 gallon Product Oil and 1000 gallon Waste Oil USTs <br />were removed. Soil samples were collected from the fill material around the <br />USTs and the soil stockpile generated during the UST removal activities. <br />Analytical results of those soil samples were non -detect for contaminants. <br />PHSIEHD does not consider those samples representative of the soil beneath <br />the cement slab where the USTs were located. No soil samples were collected <br />beneath the cement slab that was beneath the USTs as required by PHSIEHD. <br />A PHSIEHD staff report, dated July 21, 1999, notes that 130 feet of piping was <br />removed without PHSIEHD inspection and that soil samples were not collected <br />beneath this piping. According to a map of this facility, previously provided to <br />PHSIEHD, piping associated with the -Waste Oil and Product Oil USTs still exists <br />beneath the building at this site. This piping must be'�closed in place under <br />PHSIEHD permit and inspection. <br />.l <br />A plan to investigate the lateral and vertical extent ofIthe documented <br />contamination at this site and investigate the areas previously not properly <br />addressed. should be submitted to PHSIEHD by March 1, 2000. In addition, a <br />plan for closure of the piping beneath the building should bye submitted that same <br />date.
