Verification of proper Bravo Conversion Frame installation. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />5. Make a hazardous waste determination of any waste generated including the removed <br />equipment, and manage it accordingly. Within 30 days of completion of the proposed <br />work, submit to EHD document detailing how the waste is managed, and a copy of the <br />disposal record if the waste is to be removed off site in accordance to the hazardous <br />waste determination. <br />6. Log into the California Environmental Reporting System (CERS), update and submit the <br />"UST Monitoring Plan" for the new converted diesel tank for approval. <br />7. Log into the California Environmental Reporting System (CERS), complete and submit a <br />"UST Certification of Installation/Modification" form for the new converted diesel tank. <br />8. All components must be certified by an independent testing organization and labeled as <br />required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br />Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br />9. Permit expires 180 days from the date of approval.
