No site records were <br /> available for the alarms on 1/8/18 (1-8 87 STP sump) and 11/16/17 (1-5 UDC 9/10 and 1-8). The designated operator <br /> reports stated these alarms were addressed by the site owner or site manager removing rain water. According to <br /> Mohammad Hussaini, the site manager, the site does not open the sumps for any reason and has never removed <br /> liquid. During the monthly inspection, the designated operator shall review the alarm history for the previous month, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> The designated operator documented that all tank top sumps were visually inspected the for leaks, water, or debris <br /> during the every monthly inspection. The site manager stated that he has only witnessed the UDCs being inspected. <br /> During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and <br /> any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators <br /> performing monthly inspections at this facility are inspecting all of the required areas and noting it accurately on the <br /> reports. <br /> This is a minor violation. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The fuel filters in the five dispensers on site are not contained within the under dispenser containment(UDCs). The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately take all necessary <br /> steps to ensure any drips or leakage from fuel filters will drain into the UDCs. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 91 fill sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring of <br /> the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br /> of product at the dispenser when it detects a leak. The sensor was shaken by the technician several times and <br /> retested during the inspection. <br /> This is a minor violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters in the five dispensers on site are not contained within the under dispenser containment(UDCs) and do <br /> not have secondary containment. All primary portions of the UST system must be provided with secondary <br /> containment.
