Unified <br />JUL 2 8 2014 <br />Distribution QaatP ,;� .5,t4ckton <br />TH <br />OVERVIEW Emergency Notification for Spills and RAWATAMEfVT <br />In the event of a release or threatened release (regardless of quantity) of hazardous <br />materials (Diesel fuel, Anhydrous Ammonia, Sulfuric Acid, Sodium Hypochlorite (bleach), Freon <br />R-22) various environmental agencies must be notified. This document will assist you in <br />determining the correct steps of communication and which agencies to notify based upon the <br />scenario. <br />1. FIRST ON SCENE <br />First on scene should immediately notify the maintenance department. The maintenance <br />department will respond to the scene and assess the situation. Once the situation has <br />been assessed, the maintenance department will immediately contact <br />3E @ 800-451-8346 <br />THE CALLER SHOULD BE PREPARED TO PROVIDE THE FOLLOWING <br />INFORMATION <br />1. Nature of emergency — <br />WHAT HAS HAPPENED? <br />WHAT WAS SPILLED OR RELEASED? <br />HOW MUCH WAS SPILLED OR RELEASED? <br />WHAT TIME THE SPILL OCCURRED? <br />2. Address — <br />SITE ADDRESS. <br />3. Phone number — <br />CONTACT TELEPHONE NUMBER? <br />4. On site location — <br />WHERE IS THE SPILL OR RELEASE LOCATED ON PROPERTY? <br />5. What has been done? <br />THE SPILL HAS BEEN CONTAINED OR CLEANED UP, EVERYTHING IS UNDER <br />CONTROL; or <br />WE HAVE STOPPED AND CONTAINED THE SPILL; HOWEVER THE SPILL IS <br />BEYOND OUR CAPABILITES, DISPATCH EMERGENCY RESPONSE TEAM; or <br />THE SPILL IS NOT CONTAINED, DISPATCH EMERGENCY RESPONSE TEAM. <br />6.
