An emergency coordinator and modified contingency plan information was not posted by the facility phone but stored in the Hazmat binder. The person(John Doucette) listed as the emergency coordinator on this form is no longer current. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. During the inspection, the required information was posted by the facility phone. An addition form was provided that can be used for this purpose. This is a repeat violation, Class Il. 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records of used oil for 2012 and 2015 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for 2012 and 2015, maintain them on site, and submit copies to the EHD. This is a minor violation. 114 CCR 66266.81(a)(4)(B) Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. The records of battery purchases did not have legible documentation to show how many battery cores were returned to the battery vendor. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading or manifests for lead acid batteries'for the last three years, maintain them on site, and submit copies to the EHD. This is a Class II violation. 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. Dark oil stains were observed on the floor of the basement and in the maintenance room behind the main shop. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean up the dark oil stains, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed.
