The person(John Doucette) listed as the emergency coordinator on this form is no longer <br /> current. There must be at least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of <br /> the fire department, unless the facility has a direct alarm. During the inspection, the required information was posted <br /> by the facility phone. An addition form was provided that can be used for this purpose. <br /> This is a repeat violation, Class Il. <br /> 112 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records of used oil for 2012 and 2015 were not found on site. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated <br /> manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of <br /> all manifests and receipts for 2012 and 2015, maintain them on site, and submit copies to the EHD. <br /> This is a minor violation. <br /> 114 CCR 66266.81(a)(4)(B) Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. <br /> The records of battery purchases did not have legible documentation to show how many battery cores were returned to <br /> the battery vendor. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a <br /> copy of all bills of lading or manifests for lead acid batteries'for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Dark oil stains were observed on the floor of the basement and in the maintenance room behind the main shop. <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean up the <br /> dark oil stains, and manage according to Title 22 hazardous waste regulations.
