A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number . <br /> Immediately reactivate your hazardous waste ID number and submit evidence to the EHD. <br /> This is a minor violation. <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> This is a minor violation. <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated (not necessarily documented)that employees who handle <br /> hazardous waste were properly trained. According to Ms. Olivarez, the main mechanic at the facility was hired <br /> within the last year and she is unsure if he received the full hazardous waste training. The generator must ensure <br /> that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and emergency <br /> procedures. Provide proof of training to the EHD for employees whose responsibilities include hazardous waste. <br /> This is a minor violation. <br /> 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> -Copies of hazardous waste disposal records for used antifreeze were not found on site for the last three years. <br /> -Copies of used oil disposal records between May 2015 and March 2017 were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all missing manifests and receipts for the last three years, maintain them on site, and <br /> submit copies to the EHD. <br /> This is a minor violation. <br /> FA0017671 PR0538614 SCO01 04/03/2018 <br /> EHD 22-01 Rev.09/22/16 Page 4 of 7 Small Quantity Hazardous Waste Generator OIR <br />
