Ship date 5/22/2012 DEC 01 2014 <br />This manifest had two line items, as follows: ENVIRONMENTAL HEALTH <br />DEPARTMENT <br />Line Item 9b 1: Pour drums of personal protective equipment contaminated <br />with lead <br />Line Item 9b2: One drum of floor sweeps <br />The four drums of PPE that were originally shipped on manifest number 009479250JJK <br />were shipped to Clean Harbors San Jose on manifest number 009479260JJK on 6/4/2012, <br />for which a signed manifest copy dated 6/8/2012 has been received. <br />The one drum of floor sweeps ultimately had to be re -profiled, a process which has taken <br />several months to complete. This drum of floor sweeps has been shipped to Clean <br />Harbors on manifest number 009476834JJK on 10/23/2012. A signed copy has been <br />received from the designated facility and a copy of the manifest is included with this <br />letter. <br />This accounts for all of the drums shipped from ECS Refining Stockton.
