All containers of hazardous waste on site have been on site since 2012 or sooner. Facilities who <br /> generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored <br /> on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous <br /> waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by <br /> February 17, 2014. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by February 17, 2014 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Note: <br /> This facility is no longer generating hazardous waste. According to Mr. Clark, this facility has not <br /> generated hazardous waste since 2012. The shop on site is being shared with Ross Roberts Trucking. <br /> The following containers of hazardous waste were observed on site, noted as belonging to Blackmun <br /> Equipment Leasing from prior to Ross Roberts Trucking moving into the shop, and must be removed by <br /> a properly licensed hazardous waste hauler in order to complete the closure: <br /> - a 55 gallon blue plastic drum which previously held hazardous waste liquids <br /> - a 55 gallon white plastic drum which previously held hazardous waste liquids <br /> -two 55 gallon black steel drums containing an unknown black sludge <br /> -three 5 gallon blue plastic buckets of oily liquid <br /> - a 30 gallon white plastic drum labeled only "transfer fuel container BAD" <br /> Submit copies of the disposal records for these wastes to the EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT IME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Receiv Title: <br /> Stacy Rivera (209) 468-3440 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
