At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle hazardous waste were properly trained. According to Mr. Bermingham, hazardous waste training is not being conducted. The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. Immediately provide training to all employees who handle hazardous waste and submit a copy of the training records to the EHD by June 5, 2011. 60, 61, & 62. Manifest number 003774WJJK (7/10/08) was not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. By June 5, 2011, submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. i i 84. Facility was unable to demonstrate the amount of time electronic wastes were stored on site. A universal waste generator shall accumulate universal waste for no longer than one year from the date the waste was generated. Immediately dispose of this waste as universal waste and submit a copy of the disposal record to the EHD by June 5, 2011. Complete and submit a copy of the Return to Compliance Certification to the EHD by June 5, 2011 along with a statement documenting the corrective actions that have been or will be taken for each violation and any supporting paperwork.
