SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH DEPARTMENT <br />600 East Main Street, Stockton, California 95202-3029 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.si4ov_orglehd/unitiii.html <br />CONTINUATION FORM <br />Page: 4 of b <br />OFFICIAL INSPECTION REPORT <br />Date: 03/14/11 <br />Facility Address: 3 B's Truck and Auto Plaza 14749 N Thornton Rd Lodi CA 95241 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />amend that plan to incorporate hazardous waste management provisions that are sufficient to comply <br />with the requirements of this chapter. <br />(c) The plan shall describe arrangements agreed to by local police departments, fire departments, <br />hospitals, contractors, and State and local emergency response teams to coordinate emergency <br />services pursuant to section 66265.37. <br />(d) The pian shall list names, addresses, and phone numbers (office and home) of all persons qualified <br />to act as emergency coordinator (see section 66265.55), and this list shall be kept up to date. Where <br />more than one person is listed, one shall be named as primary emergency coordinator and others shall <br />be listed in the order in which they will assume responsibility as alternates. <br />(e) The plan shall include a list of all emergency equipment at the facility (such as fire extinguishing <br />systems, spill control equipment, communications and alarm systems (internal and external), and <br />decontamination equipment), where this equipment is required. This list shall be kept up to date. In <br />addition, the plan shall include the location and a physical description of each item on the list, and a brief <br />outline of its capabilities. <br />(f) The plan shall include an evacuation plan for facility personnel where there is a possibility that <br />evacuation could be necessary. This plan shall describe signal(s) to be used to begin evacuation, <br />evacuation routes, and alternate evacuation routes (in cases where the primary routes could be blocked <br />by releases of hazardous waste or fires). <br />(g) The plan shall include the current telephone number of the State Office of Emergency Services. <br />Correct immediately by updating your contingency plan to include all of the required information. <br />Class II - repeat violation. <br />49 Personnel Training records are incomplete. The owner or operator shall maintain the fallowing <br />documents and records at the facility: (1) the job title for each position at the facility related to hazardous <br />waste management, and the name of the employee filling each job; (2) a written job description for each <br />position listed including the requisite skill, education, or other qualifications and duties; (3) a written job <br />description of the type and amount of both introductory and continuing training that will be given; (4) <br />records that document that the training or job experience required has been given to, and completed by <br />facility personnel. Correct immediately by ensuring that the required training and documentation are <br />completed and available for review. Submit a copy of the required documentation of training to this office <br />by 4-14-11.
