SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR -Notice to Co Routine Hazardous Waste Inspection Minor/ Notice to Comply violations #: 108, 808 Repeat/ Class II violations #: 605, 305 108. Failed to determine if waste is a hazardous waste. Two boxes full of ballast, both stored upstairs, one in the old equipment room and the other in adjacent to the front office/ storage of office files, have been removed from the light housing unit. It is unclear if there is a future need for the ballast. There is a small bottle without cap of 1/4 full of dried Video Jet Ink that states it is flammable item found upstairs in the old equipment room. Any person who generates a waste shall determine if the waste is a hazardous waste. Make a hazardous waste determination for the ballast and Video Jet Ink, and manage them according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. 605. Failed to completely label containers of hazardous waste. Three 55 gallon metal drums full of oily solids located at the outdoor hazardous chemical storage area which all lacked type of hazardous property. All hazardous waste containers shall be marked with the following information: 1. the words "Hazardous Waste", 2. name and address of generator, 3. hazardous properties 4. physical state, 5. composition (contents), 6. accumulation start date Immediately label these containers and ensure that all containers are marked with all the required information. - Corrected on site. Safety Leader marked the hazardous property. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
